As long as you mean type "blog.php" in the custom urls and leave
the default as plain urls, that is okay with me. I think making custom
URLs the default is asking for trouble, since so many people have problems
with it.
Also, putting blog.php as the default makes custom urls look
initially less desirable, I think.
